Details of Experience:
Mr. Medina Augustin was admitted to The Grand Pavilion for Rehabilitation and Nursing on 06/17/19 as he acquired the absence of left leg below the knee and in the need of intense rehabilitation therapy. Mr. Augustin presented to The Grand Pavilion with a diagnosis of Radial nerve palsy (wrist weakness) and generalized muscle weakness.

Upon his admission to The Grand Pavilion, Mr. Augustin was greeted by the community’s staff, assessed and made comfortable. Therapy spent a great deal of time discussing his treatment goals and plans to get him back on his feet and a return to his normal activities of daily living (ADL).

Upon Mr. Augustin’s admission to The Grand Pavilion, he had a decrease in leisure task participation, as well as, a decrease in his independent participation with functional activities of ADL tasks. He presented with impairments in balance, sensation, limitations and/or participation, with restrictions in completing his self-care and general tasks and demands.

Therapy worked hard to assess his needs and developed a daily plan of therapy which enhanced, encouraged and promoted skilled physical and occupational therapy services. The Therapists evaluated and supported his recovery of safety awareness, improved dynamic balance, increase functional activity tolerance and increased body strength. They helped him to minimize his fall risks and also decreased his complaints of pain while enhancing his quality of life. All of this while working alongside the motivated and determined Mr. Augustin who worked fiercely and effortlessly with the therapy team in reaching his target goals of recovery and success.

During his stay at The Grand Pavilion, he also provided great compliments regarding the welcome he received, the care, support and encouragement provided by the Grand Pavilion Staff and especially our Amputee walking school program, which is lead by our phenomenal Todd Schaffhauser and Dennis Oehler, 3-time Paralympic Gold Medalists, 1988 Seoul Paralympics, and the fastest leg amputees in the world! He stated that “With Dennis and Todd and the entire team at The Grand Pavilion, I feel like family”

As he made consistent progress throughout his treatment and care, Mr. Augustin was able to ambulate 200 feet. Mr. Augustin had a successful discharge from The Grand Pavilion on 09/26/2019 and ranked the community a 10/10. In addition to Mr.Augustin’s positive feedback, he left us a 5-star Google review.
