Details of Experience: Due to a fall, Mrs. Roberts was admitted to The Grand Pavilion on February 23rd for rehabilitation. Mrs. Roberts stated that she was getting out of the shower at home and slipped on the bathroom mat, falling on her elbow. She crawled to press the life alert device, and EMT services arrived within 15 minutes. Upon her arrival at The Grand Pavilion, Mrs. Roberts was welcomed and made comfortable by team members, including Concierge, Nursing, Rehabilitation, and Social Work.

After being greeted and evaluated, the team determined that Mrs. Roberts required intense physical and occupational therapy. Due to her fall, she fractured her left elbow and left knee and had decreased performance with ADLs (activities of daily living.) It was the staff’s pleasure to work alongside this determined patient, who strived to reach her goals. As she worked very hard to achieve those goals, it was to no one’s surprise that she was able to ambulate 150 feet up and down the hallway twice a day, independently.

Mrs. Roberts appreciated all the staff that cared for her during her stay. When asked during her Concierge evaluation about her experience at The Grand Pavilion, she stated, “They are very nice. The therapy is wonderful. I worked with Katelyn and Cornelia, and I couldn’t ask for anything better. They are terrific therapists. A special shoutout to my nurse’s aides, Bibi, Sharon, and Priya”. The Grand Pavilion is delighted that she returned home safely to her family on March 19th of 2021.

Details of Experience:
Mr. Francis was admitted to The Grand Pavilion on January 28th, for rehabilitation due to multiple falls caused by his Parkinson’s disease. Upon his arrival to the Grand Pavilion, Mr. Francis was welcomed and made comfortable by members of our team including: Concierge, Nursing, Rehab and Social Work. After being in the hospital for a while, Mr. Francis was looking forward to beginning the process of Rehabilitation.

The patient was greeted and evaluated by our dedicated therapy team, where it was determined that Mr. Francis was in need of intense physical and occupational therapy. The patient had decreased performance with ADL’S (activities of daily living) and impaired balance. It was the staff’s pleasure to work alongside this determined patient, who strived to reach his goals. As he worked very hard to reach those goals, it was to no one’s surprise that he was enjoying great progress. After a short period of time working very hard and focused on his goals he was able to ambulate independently.

Mr. Francis appreciated all the staff that cared for him and rated us a 10/10. When asked during his Concierge evaluation if he would recommend the Grand Pavilion to a friend or family member, he stated: “yes, very much so and I would come back if need be.” Mr. Francis also thanked his rehab therapists, for always taking care of his needs and especially his speech therapist Kelly Shoemaker for being there for him. The Grand Pavilion is delighted that he returned home safely to his family on February 26th of 2021.

Details of Experience:
Mr. Bilek was admitted to St. Joseph’s Hospital on November 30th due to a diabetic foot ulcer (serious bacterial skin infection). He was then admitted to The Grand Pavilion on December 4th. Upon his arrival to the Grand Pavilion, Mr. Bilek was welcomed and made comfortable by members of the team including Concierge, Nursing, Rehab, and Social Work. This was not his first stay at The Grand Pavilion, and he was delighted to come back to get better and stronger and be able to return home safely. The patient was greeted and evaluated by our dedicated therapy team, who determined that Mrs. Bilek was in the need of intense physical and occupational therapy. Due to his current status, the patient had decreased performance with ADLs (activities of daily living).

It was the staff’s pleasure to work alongside this determined, self-motivated patient, who strove to reach his goals. As he worked very hard to reach his goals, it was to no one’s surprise that he was enjoying great progress. After a short period of time working consistently with his therapists, he was able to ambulate independently. Even during his time on the Covid unit, he was not the type of guy to give up. On the contrary, he worked even harder and harder to reach his goals to be able to return to his family who was rooting for him.

Mr. Bilek very much appreciated all the staff that cared for him and rated the community 10/10 and recommended us to a friend of his that he works out with at a local gym. Mr. Bilek thanked his Rehab therapists, Mike, Flora, Melvin, Marvin, and Katelyn (the January Employee of the month) for always taking care of his needs and assisting him with building him up by strengthening his arms and legs. Mrs. Bilek stated that “Everybody has been very responsive!” The Grand Pavilion is delighted that he is returning home safely to his loving family.

Upon her arrival to The Grand Pavilion, Mrs. Smith was welcomed and made comfortable by members of The Grand Pavilion including the concierge, nursing, rehab, and social work teams. This was not Mrs. Smith’s first stay at The Grand Pavilion, she was a resident here in 2016 and was delighted to come back here to get better and stronger to be able to return home safely.

The patient was greeted and evaluated by the dedicated therapy team where it was determined that Mrs. Smith was in the need of intense physical and occupational therapy. Due to her fall, the patient had decreased performance with ADLs (activities of daily living). Mrs. Smith presented with lower back pain limiting her ability to stand for prolonged periods. It was the staff’s pleasure to work alongside this determined, self-motivated patient, who strived to reach her goals.

As she worked very hard to reach her goals, it was to no one’s surprise that she was enjoying great progress. After a short time working consistently with her therapists, she was able to ambulate 75 ft. with assistance. During her concierge evaluation. Mrs. Smith shared that: “There was a member of the maintenance team that I call my Guardian Angel. When I first arrived on the third floor I was beside myself. This gentleman, Denver, talked to me until I could relax”.

As Mrs. Smith worked diligently, it was wonderful to share in her progress. Mrs. Smith very much appreciated all the staff that cared for her and went out of her way to thank her rehab therapists, Sofia, Rebecca, Debbie, Serena, and Olga for always taking care of her needs and making her feel comfortable. The Grand Pavilion is delighted that she is returning home safely.

Lila Fischer was admitted to South Nassau Hospital on August 13th, 2020 due to a fall on concrete floor, which caused her to fracture her pelvis and multiple ribs. She was then admitted to The Grand Pavilion on August 24th. Upon her arrival to the Grand Pavilion, Mrs. Fischer was welcomed and made comfortable by members of our clinical team including: Concierge, Nursing, Rehab and Social Work.

The patient was greeted and evaluated by our dedicated therapy team, where it was determined that Mrs. Fischer was in the need of intense physical and occupational therapy. Due to her fall, the patient had decreased performance with ADL’S (activities of daily living),. Mrs. Fischer presented with lower back pain, limiting ability to stand for prolonged periods of time. As much as Mrs. Fischer misses her home and community she is very happy at The Grand Pavilion. She mentioned that she is being treated with respect and dignity by the dedicated staff. Mrs. Fischer stated, “I really appreciate their kindness. Mariam went out of her way for me, and she is so kind.”

As she worked very hard to reach her goals, it was to no one’s surprise that she was enjoying great progress. After a short period of time working consistently with her therapists, she was able to ambulate 200 ft independently. During her Concierge evaluation, Mrs. Fischer was in tears, as she remembered the day she came into The Grand Pavilion and was unable to raise herself from the bed without the assistance of two staff members. She also stated how she loved working with her physical and occupational therapists Marian and Sofia.

Mrs. Fischer very much appreciated all the staff that cared for her and rated us a 10/10. Mrs. Fischer went out of her way to thank her Nursing Aides, Valentine, Sharon, Bibi(1st fl), Bibi(2nd fl), and Sharon for always taking care of her needs and making her feel comfortable. The Grand Pavilion is delighted that she is returning home safely.

Mrs. Harris was admitted to Mount Sinai Hospital on 05/31/2020 due to Subarachnoid Hemorrhage (stroke) that was caused by bleeding in the subarachnoid space surrounding her brain. She was then admitted to The Grand Pavilion on July 29th. Upon her arrival at the Grand Pavilion, Mrs. Harris was welcomed and made comfortable by members of our clinical team including: Concierge, Nursing, Rehabilitation and Social Work.

The patient was greeted and evaluated by our dedicated therapy team, where it was determined that Mrs. Harris was in the need of intense physical, occupational and speech therapy. Due to her stroke the patient had decreased performance with ADL’S (activities of daily living), as well as, low volume of speech while communicating her needs to the staff. Mrs. Harris presented with impaired sit and stand balance, decreased functional activity, tolerance and endurance.

As much as Mrs. Harris misses her home and community, she is very happy at The Grand Pavilion. She mentioned that she is being treated with respect and dignity by the staff, as a patient deserves and only has good things to say about her nurse aides. Mrs. Harris took the time to thank the recreation team for always facilitating window and virtual visits between herself and her family, and to be able to see them during these challenging times. She is a family oriented person and loved by all.

As she worked very hard to reach her goals, it was to no one’s surprise that she was enjoying great progress. After a short time working consistently with her therapists, Mrs. Harris was able to communicate and speak properly and that she was no longer in need of speech therapy. Her Speech Therapist, Kelly Shoemaker, mentioned how well she progressed and how it was a pleasure to work with her and is very proud of the goals she reached. Mrs. Harris stated how she still loves working with her physical and occupational therapists Debbie and Flora, and hopes she will soon be able to return to the community that she misses very much.

The Grand Pavilion staff is assisting her with discharge plans, as she is getting closer to being discharged and heading back home. The staff prays for her continued health and is very proud of the challenges she conquered and the goals that she reached.

While preparing for discharge, Mary remembered the Heart Health event she attended at The Grand Pavilion for Rehabilitation and Nursing at Rockville Centre in February 2020 and decided to admit to the community. Upon her arrival to the community on August 14th, Mary was welcomed and made comfortable by members of our clinical team including: Concierge, Nursing, Rehabilitation and Social Work.

Mary was greeted and evaluated by our rehabilitation team which determined she was in the need of intense physical and occupational therapy. Mary had decreased performance with Activities of Daily Living (ADL) and was not able to ambulate safely. The team was honored to work alongside this determined, self-motivated patient. Mary worked diligently with her team, increasing her balance, strength, and length of ambulation every week. No matter how challenged she was, Mary always maintained her focus on her goals.

As she prepared to go home, Mary rated her stay as a 10 out of 10. She shared “The Rehabilitation team was very good… I had Katelyn and Olga; it was excellent.” In addition, she had high praise for her nursing team. Mary thanked her CNAs Joyce and Elizabeth “for always taking care of her needs and making her feel comfortable.”

Michael Costa was admitted to The Grand Pavilion on June 9, 2020, due to a fracture of the left tibia, generalized muscle weakness and difficulty walking. Upon arrival to The Grand Pavilion, Mr. Costa was welcomed and made comfortable by members of our clinical team including: Concierge, Nursing, Rehab and Social Work.

The patient was evaluated by our dedicated therapy team who determined that Mr. Costa was in the need of intense physical and occupational therapy. The patient had decreased performance with ADL’S (activities of daily living), was not able to ambulate safely and had decreased muscle strength.

The Rehabilitation team developed a customized treatment plan for Mr. Costa, including short-term and long-term goals. These goals included ambulation, bed mobility and hygiene / grooming tasks, all with minimal assistance. Mr. Costa always had something positive to say to both staff and his fellow patients. Mr. Costa stated, “The people here are great; they work very hard and I wish I can do more for the staff.” Mr. Costa also demonstrated appreciation for his nursing team, including Bibi, his Certified Nursing Assistant. Mr. Costa shared that Bibi made him feel comfortable from the beginning of his stay and he is truly grateful for her support.
