Reason for Stay: Rehab stay to regain strength after hospitalization due to heart failure

Mrs. Byrd was admitted to South Nassau Communities Hospital on November 19, 2024, for heart failure. During her stay, her hospital care plan team concluded that inpatient subacute rehab would be the best course of action for her post-hospitalization so she could regain her strength. Along with her husband, Paul, Mrs. Byrd, and her care team determined that Rockville Skilled Nursing and Rehab Center would be the best place for her to receive rehab on her road to recovery.

When Mrs. Byrd arrived at Rockville, the Director of Concierge Services, Kevin Welge, greeted her, who was working reception that day. When Mrs. Byrd arrived in her room, she was welcomed by the nursing supervisor, who helped her settle into her newly renovated surroundings.

Upon beginning her rehab, Mrs. Byrd could not get in and out of bed or put her shoes on without assistance. Due to this, the rehab staff concluded that working towards these goals would be a great path forward for Mrs. Byrd in her rehab.

Through hard work, Mrs. Byrd can now get in and out of bed with minimal assistance and can put her shoes on by herself, with a staff member only supervising for minimal assistance. She can now even roll her wheelchair by herself by pushing on the wheels.

Mrs. Byrd and her family are incredibly proud of her progress in just a few short weeks and will forever be grateful to the incredible staff at Rockville for all the hard work and help provided. We wish Mrs. Byrd continued good health and rehabilitation as she begins to feel a new sense of renewal upon returning home.
