Reason for Stay: Syncope & Collapse/Hospitalized from a fall and fracture of neck
How did this patient hear about Grand Pavilion? Family was looking for well care physical/occupational therapy, SNCH insisted GP
Patient came to Grand Pavilion after a bad fall while she was at home. While she was in the hospital, family member came in for a tour and was pleased seeing how therapy and staff was on point. When the Pt. arrived at GP, she required total care. She was unable to communicate (Spanish) with nursing staff and PT, and needed assistance at all times.
During her stay here, her condition has improved. She is now independent with walking, eating, going to the bathroom and joining recreation activities. She receives very good care from all staff in the facility including family that comes to visit every day to help her remember (dementia). Pt. and family decided to stay at the Grand Pavilion for Long Term Care. Each day she’s very active and interested in learning English from our recreation team.
